To provide you with the best care I will maintain the privacy of your health information as required by law. I understand that you have accepted my policies and during this uncertain time of COVID -19. In order to protect the health and wellbeing of everybody, I will only be providing Teletherapy via KLARA.
CONTACTING ME
I can be reached through my secure HIPPA compliant messaging platform KLARA. This method will be utilized to protect the confidentiality of your personal health information. If you click the “message us” button found at the bottom of this page we will know that we are using the most secure form of communication. I can be reached on my cell phone at 914-329-1780.
If I am on vacation, the name and contact information of the covering doctor can be found by listening to my outgoing voicemail message. In case of an emergency call 911 or go to the closest emergency room.
FEES
Full payment is due at the time of each session, unless we have made special arrangements. I accept virtual payment via protected platforms (Zelle, Venmo, Google Pay, Paypal). Understand that I have your permission to charge your credit card for cancellations made for less than 48 hours and balances that are over 30 days past due.
The current fee for sessions is $400 and all fees are subject to change. Of course, I will take your financial situation into account when setting your fee due to special circumstances. I have always been committed to helping everybody to secure affordable treatment. I am happy to provide a statement that you can submit to your insurance company for out of network reimbursement.
CANCELLATIONS
Appointments cancelled less than 48 hours prior to the scheduled time will be charged at full fee. I intentionally do not participate in any insurance plans. The high quality service that you deserve will not be dictated by your insurance company. I am your doctor and I only work for you. I will not compromise on the length of our visits, the duration of treatment and your personal privacy and confidentiality.
